  • New Market Realities for 2021 Expected to Drive Revisions of Fiscal Terms to Improve Competitiveness #rwanda #RwOT

Many African governments will take steps to adjust the fiscal regimes in 2021 to improve competitiveness

JOHANNESBURG, South Africa, November 24, 2020/ — Projected market conditions for 2021 do not indicate a return to high commodity prices, implying that the super profit era of petroleum is over; The industry's cost base has been adjusted, but African fiscal regimes are often lagging behind and remain uncompetitive in this new environment; Many African governments will take steps to adjust the fiscal regimes in 2021 to improve competitiveness.

Without bold fiscal reforms, Africa is doomed to further loose its global market share of oil & gas investment and production. This is a major takeaway, if not not the most important takeaway, from the Africa Energy Outlook 2021 released this month by the African Energy Chamber. As oil prices settle around the $60/bbl threshold in a few years, the end of the super-profit era is over and adopting competitive fiscal frameworks will become increasingly central to maintaining investment in the sector.

However, changing fiscal regimes do come with challenges. It causes uncertainty, it distorts the expected revenue profile for the state, and it is hard to arrive at what the fiscal parameters should be. Capital is nevertheless fungible, and to attract capital to possible future projects, African nations with petroleum resources will most likely have to adapt their fiscal regimes similar to how other nations have adapted them in light of the current era. Failing to do so can lead to stranded resources and outcompeted projects.

Across the continent, major projects are unlikely to be sanctioned because of challenging market dynamics, but especially because of uncompetitive fiscal regimes. The passing of the Deep Offshore and Inland Basin Production Sharing Contract (Amendment) Act, 2019 in Nigeria is an unfortunate example. The Amendment Act introduced in 2019 a combined production and price-based royalty system to replace the existing production-based royalty system, which varies according to areas of operations. While the Amendment Act is expected to help maximise government take from PSCs, it also renders the development of deepwater fields less economical. As a result, multi-billion dollar projects in Nigerian deep water are now back on the shelves because uneconomical under such fiscal terms. They notably include Owowo (ExxonMobil), Bonga South West Aparo (Shell), Bosi (ExxonMobil), Nsiko (Chevron), Preowei (Total) or Uge (ExxonMobil). Regardless of how advanced each of these pre-FID projects were, their sanctioning will not happen unless revisions are brought to the Nigerian deep water fiscal environment. Only the passing of the Petroleum Industry Bill (PIB) could now pave the way for the sanctioning before the end of the decade.

Elsewhere, market-driven fiscal reforms have brought opposite results and currently maintaining momentum in upstream activity. Angola for instance has been promoting a much stronger enabling environment since 2017. The country has completely overhauled its regulatory framework, especially to encourage investments in gas and marginal fields. Such measures have supported the sanctioning of several deep water subsea tie back projects by international majors, confirming the direct link between attractive fiscal terms and investments. These notably include Block 17 and 32 (Total), Block 31 (BP), Block 15/06 (Eni), Block 15 (ExxonMobil) and Block 14 (Chevron).

Within its 2021 Outlook, the Chamber notably demonstrates how the adoption of a UK-type fiscal regime can help unlock $100 billion investments in Africa, even in a $50/bbl scenario. The same reforms could result in a production increase of up to 1 million barrels per day by 2030 on the continent. Such achievements and their impact on macro-economic stability and jobs creation cannot be under-estimated as African nations seeks efficient ways to recover from the Covid-19 pandemic.

  • RCSI and BD partner to expand access to paediatric surgery in Sub-Saharan Africa #rwanda #RwOT

DUBLIN, Nov. 24, 2020 — The RCSI Institute of Global Surgery and global medical technology company, BD (Becton, Dickinson and Company) (NYSE: BDX), today announce KidSURG, a joint initiative aimed at significantly improving paediatric surgical services across Southern Malawi.

Created in collaboration with leading Malawian paediatric surgeon,Professor Eric Borgstein, the KidSURG initiative will develop a paediatric surgical network in Southern Malawi, to expand surgical access to 8 million children. The initiative will also seek to develop and share optimal models for replicating this initiative in other countries. BD has donated $500,000 in cash and surgical products to support the initiative.

Malawi is a region within Africa that faces a catastrophic deficit in surgical care access. The country's unmet need for surgical care is estimated to be 41 million cases per year, resulting in a greater number of deaths than those resulting from TB, HIV and Malaria combined.

Through the KidSURG initiative, RCSI worked in partnership with Professor Borgstein and his team in Malawi to identify the greatest challenges when it comes to expanding surgical access. Together they will work to reduce unnecessary childhood mortality by addressing the shortfall in surgical equipment and medical expertise within district hospitals in the region. The KidSURG initiative will train healthcare workers to deliver high-quality, pre-referral care for complex paediatric surgical patients, and deliver safe surgery for some of the most common surgical procedures needed by children.

The initiative will also direct the development of a specialist mentor network, through which paediatric surgeons in central hospitals can provide immediate advice on patient management to regional district hospitals. Additionally, the project will supply custom-made paediatric surgical toolboxes, sponsored by BD, providing critical equipment needed to operate safely on children.

Following initial roll out, RCSI will evaluate opportunities to scale-up and expand the initiative elsewhere in a cost effective manner, ensuring that its impact is maximised.

Professor Eric Borgstein, Consultant Paediatric Surgeon at the Queen Elizabeth Central Hospital, Malawi said: 'The KidSURG initiative is a powerful example of practical and effective support that will help increase expertise in district hospitals across Malawi. I welcome this important collaboration with RCSI and extend my sincere thanks to BD for supporting our critical work on the ground.'

Professor Mark Shrime, O'Brien Chair of Global Surgery at RCSI, said: 'For over ten years, RCSI has pioneered and accelerated change to provide solutions to the surgical deficit, focusing on low and middle-income countries. We are proud to announce this partnership with BD which will significantly reduce mortality rates for children.'

Greg Quinn, Director of Public Policy & Advocacy at BD, said: 'BD has strong heritage of facilitating best-practice patient and healthcare worker safety across the world. This important partnership with our RCSI colleagues and local Malawi professionals will enable the best surgical outcomes and the safest possible interventions for children in Malawi, and we also hope it will become a model for expanding safe surgical access to children in other developing nations as well.'

Building on extensive experience in surgical training, education and research partnerships in Africa, the RCSI Institute of Global Surgery works with local partners to develop sustainable surgical care systems in low and middle-income countries. The Institute has a dual focus: capacity-building to alleviate the surgical burden, and high impact research to set the foundation for greater growth.

For nine years RCSI researchers have led the COST and SURG-Africa global surgery consortia working to develop, implement and evaluate sustainable training models in which local physicians and non-physician clinicians are trained to deliver safe surgery in rural hospitals with the support of consultant surgeons from within the region.

RCSI University of Medicine and Health Sciences is ranked first in the world for 'Good Health and Well-being' in the Times Higher Education University Impact Rankings 2020. The University's leading global position in this category reflects RCSI's singular focus on improving human health, for the benefit of patients and communities across the globe.

One dosing regimen (n=2,741) showed vaccine efficacy of 90% when AZD1222 was given as a half dose, followed by a full dose at least one month apart, and another dosing regimen (n=8,895) showed 62% efficacy when given as two full doses at least one month apart. The combined analysis from both dosing regimens (n=11,636) resulted in an average efficacy of 70%.

All results were statistically significant (p<=0.0001). More data will continue to accumulate and additional analysis will be conducted, refining the efficacy reading and establishing the duration of protection.

An independent Data Safety Monitoring Board determined that the analysis met its primary endpoint showing protection from COVID-19 occurring 14 days or more after receiving two doses of the vaccine.

No serious safety events related to the vaccine have been confirmed. AZD1222 was well tolerated across both dosing regimens.

AstraZeneca will now immediately prepare regulatory submission of the data to authorities around the world that have a framework in place for conditional or early approval. The Company will seek an Emergency Use Listing from the World Health Organization for an accelerated pathway to vaccine availability in low-income countries.

In parallel, the full analysis of the interim results is being submitted for publication in a peer-reviewed journal.

Professor Andrew Pollard, Chief Investigator of the Oxford Vaccine Trial at Oxford, said: 'These  findings  show  that  we  have  an  effective  vaccine  that  will  save  many  lives. Excitingly, we've found that one of our dosing regimens may be around 90% effective and if this dosing regime is used, more people could be vaccinated with planned vaccine supply.

Today's announcement is only possible thanks  to  the  many  volunteers  in  our  trial,  and  the  hard  working  and  talented  team  of  researchers based around the world.'

Pascal Soriot, Chief Executive Officer, said: 'Today marks an important milestone in our fight against the pandemic. This vaccine's efficacy and safety confirm that it will be highly effective against COVID-19 and will have an immediate impact on this public health emergency. Furthermore, the vaccine's simple supply chain and our no-profit pledge and commitment to broad, equitable and timely access means it will be affordable and globally available, supplying hundreds of millions of doses on approval.'

The pooled analysis included data from the COV002 Phase II/III trial in the UK and COV003 Phase III trial in Brazil. Over 23,000 participants are being assessed following two doses of either a half-dose/full-dose regimen or a regimen of two full doses of AZD1222 or a comparator, meningococcal conjugate vaccine called MenACWY or saline.

The global trials are evaluating participants aged 18 years or over from diverse racial and geographic groups who are healthy or have stable underlying medical conditions.

Clinical trials are also being conducted in the US, Japan, Russia, South Africa, Kenya and Latin America with planned trials in other European and Asian countries.

In total, the Company expects to enrol up to 60,000 participants globally.

The Company is making rapid progress in manufacturing with a capacity of up to 3 billion doses of the vaccine in 2021 on a rolling basis, pending regulatory approval.

The vaccine can be stored, transported and handled at normal refrigerated conditions (2-8 degrees Celsius/ 36-46 degrees Fahrenheit) for at least six months and administered within existing healthcare settings.

AstraZeneca continues to engage with governments, multilateral organisations and collaborators around the world to ensure broad and equitable access to the vaccine at no profit for the duration of the pandemic.

    COV002

COV002 is a single-blinded, multi-centre, randomised, controlled Phase II/III trial assessing the safety, efficacy and immunogenicity of AZD1222 in 12,390 participants in the UK.

Trial participants to date are aged 18 years or over, who are healthy or have medically stable chronic diseases and are at increased risk for being exposed to the SARS-CoV-2 virus. Participants receive one or two intramuscular doses of a half dose (~2.5 x1010 viral particles) or full dose (~5×1010 viral particles) of AZD1222 or comparator, meningococcal vaccine MenACWY. Participants have blood samples drawn and clinical assessments for safety as well as immunogenicity at multiple timepoints up to one year post-vaccination. Suspected cases presenting with compatible symptoms were tested for virological confirmation by COVID-19 PCR.

In addition, weekly swabbing are done for detection of infection and assessment of vaccine efficacy against infection.

    COV003

COV003 is a single-blinded, multi-centre, randomised, controlled Phase III trial assessing the safety, efficacy, and immunogenicity of AZD1222 in 10,300 participants in Brazil. Trial participants to date are aged 18 years or over, who are healthy or have medically stable chronic diseases and are at increased risk for being exposed to the SARS-CoV-2 virus. Participants are randomised to receive two intramuscular doses of a full dose (~5×1010 viral particles) of AZD1222 or comparator, meningococcal vaccine MenACWY as first dose and a saline placebo as second dose.

Participants have blood samples drawn and clinical assessments for safety as well as immunogenicity at multiple timepoints up to one year post-vaccination.

Suspected cases presenting with compatible symptoms were tested for virological confirmation by COVID-19 PCR.

    AZD1222
AZD1222 was co-invented by the University of Oxford and its spin-out company, Vaccitech. It uses a replication-deficient chimpanzee viral vector based on a weakened version of a common cold virus (adenovirus) that causes infections in chimpanzees and contains the genetic material of the SARS-CoV-2 virus spike protein.

After vaccination, the surface spike protein is produced, priming the immune system to attack the SARS-CoV-2 virus if it later infects the body.
